It’s not something you can easily measure or quantify in our everyday lives.
In contrast to other forms of entertainment, video games are unique as they operate based on figures and numerical data. This extends to aspects like a character’s emotional or psychological state, which can be transformed into quantifiable game mechanics.
As a fan, I’ve noticed that games with sanity mechanisms tend to approach it uniquely, yet there’s a common thread: maintaining composure amidst highly tense, sometimes otherworldly circumstances.

This rising tension is represented by a growing shadow beneath their health bars.
When a character’s tension gauge reaches its maximum, their resolve is put to the ultimate test. If they fail to measure up, they’ll be stricken with an unbearable curse, causing them to act irrationally, becoming self-centered, cowardly, or incomprehensible. This only adds more pressure to our group and amplifies our stress levels.
In certain unusual situations, individuals who are put to the test can unexpectedly reach a virtuous state, conquering their fear and anxiety, and becoming noble, brave heroes. Regrettably, this virtuous state is fleeting, as personal struggles often demand immediate action to return one’s stress levels back to normal.
7. Eternal Darkness: Sanity’s Requiem
Insanity Beyond The Fourth Wall

In many games featuring sanity mechanics, they tend to maintain a more discreet approach, manifesting effects through minor hallucinations or straightforward stat reductions. However, Eternal Darkness deviates from this norm significantly. Instead of a gentle nudge, it delivers a hard blow to your character’s sanity and revels in the chaos that ensues.
In the game Eternal Darkness, there are three fundamental forces: Power, Magic, and a force known as Sanity, which is controlled by an ancient entity named Xel’lotath. Creatures from all types can slowly drain your characters’ sanity just by being near them, symbolized by a decreasing green gauge. As your sanity lessens, the game’s distinctive Sanity Effects start to occur.
As a fan, I jump right in with an unsettling blend of off-kilter camera angles and eerie sounds, gradually escalating into rooms brimming with phony trinkets. My character’s head might even pop off at one point, adding a touch of absurdity to the mix, not to mention a fake volume slider that gives the impression someone accidentally squashed the remote. It’s strangely amusing to embrace a bit of insanity, but if you let it consume you entirely, your health will slowly dwindle away.
6. Don’t Starve
Isolation Ain’t Good For You

Have you ever camped alone before? Some people think it’s enjoyable and peaceful, while others find the solitude a bit unsettling. As long as there are facilities like a cabin or something around, it’s typically okay, but there’s no escape when you’re playing Don’t Starve.
In the game Don’t Starve, it’s essential to balance your character’s Sanity alongside Health and Hunger. Essentially, maintaining good mental health involves consuming nutritious food, getting enough rest, and avoiding overly menacing or dark environments. However, you’ll often need to explore dangerous areas to find resources for cooking and crafting, which can cause some stress.
As your mental stability decreases, vague, spectral figures might start showing up in the corners of your sight. At first, these are simply hallucinations, but if your mental condition deteriorates beyond a critical point, they’ll become real and threaten your life. This is often accompanied by distorted visuals and sounds, serving as a grim reminder that you’re slipping out of sanity’s grasp.

Prolonged exposure to these abnormalities can cause psychological strain, which may lead to a breakdown if their sanity levels dip below their personal thresholds.
When an employee becomes extremely agitated or uncontrollable, the subsequent events depend on their other characteristics. Some employees may aimlessly wander about, while others might damage machinery and create anomalies. A few of them may even pursue and attack other employees or commit suicide.
4. Phasmophobia
Ghosts Chip Away At Your Mind

In many stories, evil specters target weak minds, either to drain people’s energy or for amusement. Given this, it makes sense that a ghost hunter, who does this as a job, should have a robust mental strength. Staying sharp-witted is the only method to control spirits effectively in the game Phasmophobia.
In each game round of Phasmophobia, every player maintains an unique personal sanity score that gets combined to determine a team’s overall sanity rating. Sanity depletes gradually due to encounters with the current ghost and usage of supernatural artifacts, but it can be replenished by employing rare items such as Sanity Pills.
As a team’s mental stability dwindles, ghosts tend to grow increasingly aggressive, inciting hunts and exhibiting stronger, potentially harmful powers. Certain ghosts focus on attacking players who are less mentally stable, ignoring those who maintain their composure.
3. Shadow Hearts
Random Battles Are Stressful

Regular encounters with random monsters are a typical feature in numerous Japanese Role-Playing Games (JRPGs), and they’re usually not something characters within the game world find noteworthy to discuss. However, in reality, constantly encountering otherworldly terrors lurking around every corner would likely be quite distressing.
In the original Shadow Hearts series and its follow-ups, each character possesses a Sanity Point (SP) alongside the standard Health Points (HP) and Magic Points (MP). During combat with enemies, a character’s SP progressively drops. When it reaches zero, they become enraged, squandering turns or attacking their own allies. To prevent this from happening, you must use restorative items to maintain their SP at optimal levels.
It’s intriguing that characters with past encounters involving the mysterious, such as Yuri, tend to start with a higher base SP (Stamina Points) and can handle slightly more mental strain. On the other hand, individuals like Alice who haven’t had many direct confrontations with otherworldly beings typically have a lower starting SP and are more susceptible to insanity.
2. World Of Horror
There’s More To Lose Than Health

In the World of Horror, the most straightforward method for the Old Gods to free themselves from you isn’t always a physical attack; sometimes it involves stabbing you in the back, metaphorically speaking. However, this doesn’t imply it’s the sole strategy they use. When facing unimaginable terrors, both your mind and body remain equally susceptible to harm.
Beyond monitoring your Stamina, which determines your capacity to withstand pain and wounds, it’s also crucial to watch your Reason score. Intense, frightening, surprising, or overtly supernatural occurrences can inflict damage on your Reason, gradually eroding your capacity to comprehend and cope with imagery that ordinary human minds are not designed to process.
In some battles, unusual opponents may launch attacks not only on your Stamina but also directly on your Intellect or both resources concurrently.
If your mind fully exhausts, you may become incoherent and be institutionalized in a psychiatric facility, effectively ending the game. Left without you to hinder them, the Ancient Deities can advance with their catastrophic plans for the world.
1. Dredge
What’s That On The Water?

Spending an afternoon leisurely fishing from a small float on a tranquil pond is a delightful pastime. On the other hand, commercial fishing out at sea isn’t relaxing; it’s challenging and stressful, particularly when you attempt it during the dark hours. This alone can be harrowing, but when the waters teem with horrors reminiscent of Dredge, the predicament becomes even more daunting.
In Dredge, navigating your boat in the dead of night may provoke an anxiety gauge symbolized by a jittery eyeball on the screen’s top. This anxiety can be alleviated through restful sleep and daytime travel, but regrettably, the game often compels you to work at night.
If your anxiety levels are excessively high, unexpected incidents start taking place. For instance, rocks suddenly emerge from the waters to collide with your vessel, birds descend swiftly to snatch your catch, and ominous creatures such as Night Anglers multiply.
