It’s believed that they aren’t fully moon-dwellers but have mixed lineage. This could mean that when the people of Fairy Vearth interbred with the people of Blue Star, an event that might have been considered taboo in their time, the resulting descendants were the Halfmoon people or the D. In essence, many inhabitants of the One Piece world, including Luffy, may carry Shandorian blood. To put it another way, Luffy and other D characters are half-alien to Earth dwellers.
