It’s been about ten years since a game last received it, and it doesn’t seem likely any will in the future, as publishers don’t want to risk that label. With platforms like Steam offering games for all ages, the appeal of a forbidden, adults-only title has faded. Plus, the ESRB’s standards are always changing, and some games rated M for Mature now contain content as strong as what used to earn an AO rating.
Looking back at the history of these ratings is fascinating, particularly when considering popular games that had to alter their content to avoid receiving an Adults Only (AO) rating. While most of these changes happened in smaller, less well-known games, a few major titles stand out as especially interesting. Let’s take a closer look at those now.

What Is ESRB?
The Entertainment Software Rating Board (ESRB) was created in 1994 after public concern over certain video games, such as Night Trap. It reviews games and assigns them one of five ratings: Everyone, Everyone 10+, Teen, Mature 17+, and Adults Only 18+. While game companies aren’t legally required to submit their games for rating, most do so to be able to sell them on consoles and in major stores.
What Is the AO Rating?
The Entertainment Software Rating Board (ESRB) gives an Adults-Only rating to games designed for people 18 and older. While some of these games are erotic, most receive this rating simply because they contain content that goes beyond what’s allowed for a Mature-rated game. Often, developers will change the content and resubmit the game to try and get a rating that will allow it to be sold in stores.
What Content Triggers AO?
Games usually receive an AO (Adults Only) rating due to intense violence, explicit sexual content, or real-money gambling. However, the ESRB (Entertainment Software Rating Board) carefully reviews each game individually. Simply having blood or suggestive themes doesn’t guarantee an AO rating. What truly matters is how these elements are presented. Many games include mature content but still achieve more common, marketable ratings.
What Separates M (Mature 17+) And AO?
Here are the definitions of both classifications on the ESRB site:
- Mature – Content is generally suitable for ages 17 and up. May contain intense violence, blood and gore, sexual content and/or strong language.
- Adults only – Content suitable only for adults ages 18 and up. May include prolonged scenes of intense violence, graphic sexual content, and/or gambling with real currency.
Both M (Mature) and AO (Adults Only) games can feature similar content, but the amount of that content dictates the rating. M17+ games typically include intense material alongside violence or nudity, while AO games primarily focus on those elements.
Why AO Is a Death Sentence for Console Games
Sony, Microsoft, and Xbox’s Stance On Ao Games
PlayStation does not publish Adult-Only games.
Generally, major console makers like Sony, Microsoft, and Nintendo don’t permit adult-oriented (AO) games on their platforms. Sony’s policies, as stated on their ratings page, reflect this stance, and it’s consistent across all three companies. To get a game listed on the PlayStation Store or Nintendo eShop, developers must ensure it receives a Mature 17+ rating or lower.
Just to clarify, games with an ‘Adults Only’ rating aren’t actually illegal, meaning companies aren’t required to refuse them. However, Sony, Microsoft, and Nintendo generally avoid associating their brands with this type of content.
Can AO Games Be Sold Physically?
While stores like GameStop and Walmart are allowed to sell adult-only (AO) games, they almost never do. This is because there are currently no AO games available for popular consoles like the PlayStation 5, Xbox Series X/S, or Nintendo Switch. They might find some older AO games for PC, but very few people buy physical copies of games anymore, as most players have switched to digital downloads.
Does Steam Allow AO Games?
Steam doesn’t mandate ESRB ratings for games; instead, each game is reviewed individually. The platform often releases games that, while not officially rated, would likely receive an ‘Adults Only’ (AO) rating if formally submitted to the ESRB.
Steam generally allows games with mature themes, but doesn’t host many that are explicitly adult-oriented—Hatred being a notable exception. While the store does feature games intended for adults, Steam has occasionally removed titles due to their content, and this became more frequent starting in 2025. Itch.io took a stricter approach, completely removing all adult games from its platform.

Grand Theft Auto: San Andreas
The Infamous Hot Coffee
It wasn’t surprising when Grand Theft Auto: San Andreas received an Adults Only (AO) rating. The reason was a hidden, unlockable minigame called “Hot Coffee.” Although it wasn’t meant to be played normally, players could access it using modifications. This content significantly increased the game’s mature themes, and because the files for Hot Coffee were included with the game itself – not created by fans as a separate mod – San Andreas was given the AO rating.
Interestingly, the initial rating was later removed. Rockstar quickly deleted all the ‘Hot Coffee’ content from the game, and the rating was changed to Mature. It’s notable that, considering all the controversial things players can experience in a GTA game, it was ‘Hot Coffee’ specifically that led to this change.
How To Play GTA: San Andreas
Luckily, San Andreas is relatively easy to play compared to some of the other games mentioned here. Finding the original release can be tricky though, as it’s largely been replaced by The Definitive Edition, which has a reputation for issues. While The Definitive Edition is playable, it’s not without its flaws. And, of course, the Hot Coffee mod isn’t included in any current version of San Andreas.
Should You Play GTA: San Andreas in 2026?
Many consider San Andreas to be the best Grand Theft Auto game, and one of the greatest open-world games ever made. Players accustomed to GTA 5 might find some aspects of San Andreas feel a little dated, and may not connect with it as strongly as those who played it when it first came out. Still, even today, in 2026, it remains a very enjoyable experience – far more so than GTA 3.
Singles (Or Singles: Flirt Up Your Life)
Sims But No Kids Allowed
Singles: Flirt Up Your Life is a mature-rated social simulation game where you control two characters who are roommates. The core gameplay revolves around developing their relationship, ultimately leading to a romantic connection. While the concept is straightforward and well-executed, it’s not particularly essential to play nowadays.
The standard version of the game released in North America is relatively mild, but previously, players could download an adult-only version that restored nude content. This version matched the content found in the European release. A sequel came out in 2005, but it wasn’t rated by the ESRB. Currently, it appears that Singles 2: Triple Trouble was only ever released in Europe.
How To Play Singles: Flirt Up Your Life
Similar to many of the games mentioned here, Singles: Flirt Up Your Life is now considered abandonware, meaning it’s no longer officially sold on platforms like Steam or GOG. However, it’s on GOG’s wishlist for potential future release.
Should You Play Singles: Flirt Up Your Life in 2026?
There’s really no point in playing Singles anymore. The original North American version is pretty bland, and while the AO version is also uninteresting, it at least offers something a little different. You’re better off playing any of The Sims games, or one of the many dating sims available on Steam.
Manhunt 2 (PC Edition)
Blood-Soaked Murder Spree
Many consider Manhunt 2 to be one of the most controversial video games ever made. Developed by Rockstar, known for pushing boundaries, this stealth game is extremely violent and graphic. Players take on the role of Daniel Lamb, who flees an asylum and embarks on a violent spree, brutally killing numerous characters. The game features a tiered system for violence, meaning the level of gore increases as the player progresses.
Manhunt 2 has a unique history because the developers modified it to achieve a Mature rating. They toned down the violence – making executions less graphic and removing a system that rewarded overly dramatic killings. The original, uncensored PC version was much more extreme and received significant criticism. While the console versions were rated M, the PC version went further and earned an Adults Only (AO) rating, despite all versions having the same content warnings.
How To Play Manhunt 2
Today, the only way to legally obtain Manhunt 2 is by finding a used physical copy of the console versions. It’s never been offered on digital platforms like Steam or GOG, and it doesn’t look like that will change. Rockstar Games rarely mentions the game anymore, effectively making it abandonware.
Should You Play Manhunt 2 in 2026?
I don’t think the censored version of Manhunt 2 is worth your time, but the complete PC version is a unique experience, even if you’ve played the original. While the two games are similar in some ways, they feel very different in terms of atmosphere and how the levels are designed, and Manhunt 2 is actually easier to play. We rarely see big-budget games like Manhunt 2 released these days.
Leisure Suit Larry: Magna Cum Laude Uncut And Uncensored
Larry’s Wildest PC Adventure
The Leisure Suit Larry series has always been known for its raunchy jokes and immature humor. First appearing in 1987, the games gained a solid fanbase throughout the ’90s with their often-crude and disrespectful style. While not considered masterpieces, the games were successful at what they set out to do. In 2004, the series received a visual update with the 3D adventure Magna Cum Laude.
This new version of the game offers players more freedom than previous installments, letting them explore a college campus, interact with others, and play mini-games. While it’s not particularly deep and the humor is often childish, it can still be enjoyable for players who are looking for lighthearted fun. Like many games released on multiple platforms, the adult version of Leisure Suit Larry: Magna Cum Laude is only available on PC. The PlayStation 2 and Xbox versions have been edited. If you want the original, unfiltered Leisure Suit Larry experience, the Uncut and Uncensored! edition is the way to go.

While some Leisure Suit Larry games are enjoyable, Magna Cum Laude is best avoided. It’s likely the weakest game in the series, with boring, repetitive gameplay and a poor story. The only noteworthy thing about it is its Adult Only rating, but that’s not nearly enough reason to play it.
Hatred
Mass Murder Sim
Although the Grand Theft Auto series is known for sparking controversy, Hatred is likely the most notorious single game ever made. It received widespread negative attention, not just from the media, but also from gamers themselves. While the game’s reputation on Steam has improved over time thanks to ongoing developer support—it currently has a “Very Positive” rating—it’s difficult to say how many of those reviews are genuine and how many are simply pranks.
Hatred is a top-down shooter where you play as someone committing a mass murder. While many shooters involve killing a large number of enemies, Hatred uniquely marketed itself as a simulation of this kind of violence. The game also emphasized the graphic and disturbing nature of the killings. Despite not condoning the protagonist’s actions – and even labeling him ‘The Antagonist’ – the game doesn’t offer much insightful commentary on why he’s doing what he does. It’s worth noting that the game Postal explored similar territory about ten years prior.

Should You Play Hatred in 2026?
While Hatred gets a bad rap, it’s not as terrible as some might think, especially when looking at it as a game. It feels a lot like an older Postal game, but Hotline Miami did similar things much better. There are simply a lot of stronger twin-stick shooters out there worth playing first, and that’s before you even consider the game’s controversial content.

MangaGamer has been publishing adult visual novels since 2008, typically releasing several titles each year. Thrill Kill
Vicious Fighter
The game Thrill Kill was one of the first to receive an Adults Only rating due to its violence, and because of the controversy, it was never officially released.
Paradox Development was creating a fighting game about a group of wicked killers who fought each other for the chance to be reborn by the goddess Marukka. Called Thrill Kill, the game was designed for four players, but negative attention led to its cancellation. If you’re curious what it would have been like, picture Wu-Tang: Shaolin Style but with demons and incredibly violent finishing moves – that’s a good approximation of Thrill Kill.

Should You Play Thrill Kill in 2026?
There’s a certain excitement to playing a game that never made it to store shelves, and while Thrill Kill isn’t terrible, it’s not very good either. The initial appeal fades fast, and you’ll quickly find the gameplay frustrating. The characters are also noticeably unbalanced. Honestly, you’re better off playing Wu-Tang: Shaolin Style instead.
Fahrenheit: Indigo Prophecy Director’s Cut
An AO Version That Might Or Might Not Exist
This version of the Quantic Dream game is extremely rare, and hard to find with this particular title. However, the Fahrenheit: Indigo Prophecy Director’s Cut does have an entry on the ESRB website, meaning it was submitted for rating at some point. The Director’s Cut appears to be the European release of the original game, and it includes scenes that were removed from the North American version to achieve a Mature rating. It’s worth noting that the ESRB only lists the PC version, so if this cut content became available, it wasn’t released on consoles.
I was really worried I’d never be able to play the original, uncensored version of Indigo Prophecy, but I was relieved to find out that’s not true! Aspyr remastered the game back in 2015, and thankfully, this version – which they call Fahrenheit – is based on the international release, meaning it’s completely uncut. It’s kind of strange, though – even with all the same mature content as the Director’s Cut, the ESRB only gave this remaster an M17+ rating.
