In a recent gathering where fans could ask questions, manga creator Gege Akutami disclosed that the main reason Gojo lost was because he let his guard down against the King of Curses. This is what Gege revealed:

Following the defeat of Mahoraga, Gojo believed that Sukuna could no longer find a loophole to circumvent his Infinity again. However, it seems to me that the usual prudent Gojo would have picked up on something being off and possibly managed to dodge a lethal injury. — Translation by Gege Akutami
