
Once you’ve finished a few Myths in the Warring States region of Nioh 3, you can start the “Headaches” Myth. Tokugawa Ieyasu will ask for your help with his terrible headaches while you’re at Hamamatsu Castle. He’ll send you to investigate a village along the Futamata Road to find medicine. This guide provides a complete walkthrough for this side quest in Nioh 3.
Headaches Myth Guide in Nioh 3
After arriving at the village, defeat any enemies you encounter and go into a house to find a body to search. Looting the body will give you the recipe for Ghostly Call. Keep in mind this is just the recipe, not the medicine itself, so you can now return to Hamamatsu Castle.
Head to the top of the castle and speak with Hattori Hanzo, who will be with Ieyasu. He’ll tell you he’s still experiencing headaches and that some Yokai have entered the castle.
Once you’re done talking, go find Nuppeppo at the castle gate. When it sees you, it will start rolling away, and all you need to do is follow it until it reaches a dead end. It’s near the back of the castle, by a wall, where it will become aggressive and the boss fight will start.
You can choose to fight the boss with either combat style, based on how much you’ve invested in developing it. The boss’s attacks are generally simple to block, making the Samurai style particularly effective. Defeating the boss will reward you with a Yokai Gallstone.

Bring the Gallstone to the Rift and find the Blacksmith. There, you can Forge Tools, which will unlock a new crafting recipe. Once you’ve crafted the Ghostly Call, return to Hamamatsu Castle and deliver the medicine to finish the quest.
