In Escape the Backrooms, surviving isn’t just about avoiding monsters and finding a way out—it’s about maintaining your mental state. This horror game differs from many others because it doesn’t use a typical health bar. Instead, your Sanity is key to survival. If your Sanity drops too low, you’ll start seeing things that aren’t there, reality will become distorted, and eventually, your character will succumb to madness, leading to death. Keep reading to discover how to recover your Sanity and stay alive in Escape the Backrooms.
What is Sanity in Escape the Backrooms?

You can check your Sanity by opening your inventory and looking at the brain icon on the right. It decreases whenever a light goes out or you hear echoes in the yellow hallways. If the brain icon starts to look warped or strange, that means your Sanity is getting low.
Each level affects your mental state differently. Generally, the darker and scarier a level is, the faster your sanity will decrease. Some levels cause sanity to drop very quickly, but others, like Level 0, are much easier on your mind and barely drain your sanity at all.
Throughout some levels, you’ll discover computer files detailing how much sanity you’ll lose in each specific area.
As a huge fan of atmospheric horror, I’ve learned that in this game, if your sanity completely runs out – that meter goes to zero – your character basically loses it. You’ll get sent back to the main menu or the last place you saved. Now, playing with friends adds another layer of tension! Everyone has their own sanity to manage, and on harder settings, if one person cracks, it can start a chain reaction and pull the whole team down with them. It really makes you look out for each other!
How to Restore Sanity in Escape the Backrooms

The best way to regain your mental state is with Almond Water. Each bottle will restore about half of your sanity, so it’s really important to stock up. You can usually find it by searching office rooms, filing cabinets, and shelves, particularly in the yellow offices or hotel areas at the beginning of the game.
It’s tempting to use Almond Water right away, but save it for when you really need it – when your health drops below half. Using it when you’re already doing well is a waste, especially later in the game when recovery is harder to come by.
In addition to Almond Water, you can also regain sanity by drinking Juice or eating Energy Bars. While not as effective, they can be lifesavers when your sanity is low.
Sanity also behaves differently depending on how hard you’re playing the game. On the hardest difficulty, Nightmare, your sanity can decrease much faster – up to 150% the normal rate. But on the easiest difficulty, Easy, you won’t lose any sanity at all.
