Once you’ve beaten the High Priest on Mount Karama in Nioh 3, you’ll need to head south and cleanse the three barriers found throughout the Heian regions. While exploring these areas and collecting items, you’ll discover a spirit hidden inside a house in Sagano.

Start at the Shrine in Sagano and head west until you find a house. Inside, you’ll see a spirit standing over a deceased woman. Speak with the spirit, and she’ll share a tale of a rumor: eating mermaid flesh is said to grant an 800-year lifespan. Her sons went searching for it, but they never returned. You need to continue their search eastward.

There is an enemy base on the way to your objective that you can clear out for additional rewards.

When you reach the search area, you’ll encounter bandits and a Yokai near some bodies. Defeat them and search the bodies to learn that the brothers who were here fought with these bandits and escaped. Continue south into Nishiichi, where you’ll find a One-eyed Oni battling bandits. Take care of the bandits first, then defeat the Oni.

Halfway through the battle, a new boss named Gaki will appear, and you’ll have to fight both bosses at the same time. Watch your footing, though! It’s easy to fall into the water or get knocked off the edge, and that will instantly defeat you.

After defeating those Yokai, speak to the mother to finish the quest. You’ll receive the Brothers’ Omamori and Ninja’s Locks as rewards.
