As you arrive at Shatterscarp and navigate to Thirdborn, you’ll come across Chiko, who is deeply concerned about their Uncle Keipo. Despite his reluctance to engage with others, they harbor hope that a figure like you might help change things for the better. Following a brief chat, Keipo asks you to retrieve a Leviathan Heart from a temple, which he had previously trapped in various ways. The purpose is for him to view it one last time before age takes its toll. However, if you decide to give Keipo the Leviathan Heart in Avowed, that’s your choice to make.
What happens if you give Keipo the Leviathan Heart in Avowed?
Keipo appreciates your action for providing him an opportunity to end his life before the Dreamscourge dominates, sparing him from potentially causing harm to Chiko and other residents of Thirdborn. He will acknowledge that Chiko will grieve for him, but you’ve spared them a grief that surpasses anything they could ever imagine.
To put it mildly, Chiko won’t be pleased with you at all. In fact, they might express their displeasure by saying something along the lines of “May the sand bury you, never to return to Thirdborn again!” Clearly, this is not a friendly sentiment.
The positive is that Keipo will give you Umpara’s Spear, a unique weapon of exceptional quality.

What happens if you don’t give Keipo the Leviathan Heart?
In this situation, I opted for the course of action where I informed Keipo about my awareness of his motive for desiring the Leviathan Heart, which was to use it as a deadly toxin. I declined providing him with the Leviathan Heart since I didn’t wish to contribute in any way to his demise.
He’s going to warn you that this course of action might lead to disastrous consequences for Chiko, and perhaps even for the entire Thirdborn group. It seems like we might all be in trouble now.
After stepping outside, you’ll inform Chiko that you didn’t hand him the Leviathan Heart. I proposed to keep it for safekeeping and take it somewhere distant instead, which they accepted. Furthermore, I shared with them that their Uncle Keipo wasn’t merely ill; he was afflicted with Dreamscourge and wished to end his life before becoming one of its victims.
As a fervent admirer, I can’t help but empathize with Keipo’s unwillingness to accept this situation. In my heart, I yearn for a solution that could halt it, even if such an option seems impossible. With a resolve check, I shared the harsh truth – there is no way to stop it, and we must honor his decision, regardless of our personal feelings. This predicament leaves Chiko in a difficult position, as they wish to spare Keipo from any suffering, but also don’t want him to possess the Leviathan Heart.
Subsequently, Chiko instructs you to put on Keipo’s armor. The armor was intended for Keipo but remains unused. Here are the details of the equipment, along with its specifications and enchantments:

5. Enchantment: Blessing of the Ancients (increases magic resistance by 20%)

Besides escaping the responsibility for contributing to someone’s demise, you will also obtain a significant amount of potent equipment.
Which decision gives you the best outcome in the Heart of Valor side quest?
Based on my experience playing through the game, I strongly advise against giving Keipo the Leviathan Heart in Avowed. Even though it seems like the right thing to do by allowing him to end his life before the Dreamscourge overtakes him, the benefits you’ll receive from not handing over the Leviathan Heart and informing Chiko that their uncle has a fatal illness and wanted to end his suffering, thus potentially preventing a town-wide outbreak, are significantly greater.
Additionally, by doing this, Chiko can spend more quality time with Keipo before his end of life. If you prioritize physical gains above all else, then this may be the best choice for you, though it might not provide a pleasant final experience.
As a gamer, if I feel a stronger sense of ethical duty to let Keipo decide his own fate, then by all means, hand him over the Leviathan Heart. His spear is still quite effective at this point in our game, but it didn’t quite suit my playstyle.
Avowed’s side quests may seem innocent at first glance, but they could have hidden repercussions in the future. In fact, refusing to give Keipo the Leviathan Heart might lead to consequences that could resurface later on, potentially causing more trouble for you down the road.
