Although not officially confirmed, it appears that selecting the Rust-Colored Egg door symbolizes a deterioration in James’ mental condition, furthering the narrative towards the In Water ending of the game.

Next, make your way down the lengthy hallway as Mary recounts her story. You can choose to listen to the dialogue or move past it without consequence. Either way, reach the end of the corridor to battle the final boss in Silent Hill 2, which will lead you to this particular ending.

In Water Ending, Explained

The rash conditions to reach this ending are quite challenging – failing to scrutinize Mary’s photo and letter, and letting my health drop dangerously low. It gives off an impression that James doesn’t value much his own life or anything else for that matter.

In the final scene set by water, James is seen expressing sorrow over his actions of killing Mary while seated inside his car. Overwhelmed with remorse and unable to find peace, James plunges himself into a profound despair that ultimately leads to his tragic end through suicide. Unable to bear life without Mary or the burden of guilt for ending her existence, he drives his vehicle into Toluca Lake, ending his own life.

In the chilly and damp expanse of Toluca Lake, James plunges, while Mary’s letter is recounted to the player, revealing her final message to James before she passed, expressing forgiveness and eternal love. Unable to endure life without Mary, James never learns the contents of that letter, instead letting it sink with him into Toluca Lake. It’s significant to mention that Mary’s lifeless body is in the trunk of James’ car, so by driving into Toluca Lake, he finds peace by joining Mary in death, as their bodies will rest together at the lake bottom.
