Elizabeth chooses to proceed and buys the product, needing to retrieve it from an unusual hidden spot. After injecting herself with the Substance serum, she experiences childbirth, producing a younger ‘flawless’ version of herself, whom she names Sue (Margaret Qualley). Sue sews Elizabeth up afterward. Sue is given guidelines stating they will alternate one week in each other’s bodies, and these regulations must be followed strictly. With Elizabeth unconscious on the ground, Sue seeks out the job that Elizabeth had and fits Harvey’s requirements perfectly.

When Elizabeth finds herself unwillingly swapping bodies with Sue, she grapples with persistent feelings of sadness and a lack of self-worth. The contrast between their personalities becomes more apparent as time passes, and due to the disregard for rules, Elizabeth begins noticing unusual alterations in her physical form, such as one of her fingers rapidly aging, appearing infected, and taking on a grotesque, corpse-like quality. In desperation, she reaches out to the supplier who suggests adhering to the rules or ceasing the substance; however, neither option can restore her appearance. Ultimately, Elizabeth chooses to persist with the swaps, but the situation only deteriorates further.

Sue becomes extremely popular, but disregards safety measures concerning a special serum that keeps Elizabeth young. Just before her primetime New Year’s Eve special, Sue runs out of the serum, forcing her to return Elizabeth to her original state. Elizabeth, feeling guilty for giving Sue the termination serum, tries to save Sue but regrets it instantly. In the end, an enraged Sue kills Elizabeth after a violent confrontation and goes on to host the New Year’s Eve show.

Sue is unable to give herself injections due to Elizabeth’s death, and her body rapidly deteriorates backstage. In a last ditch attempt to save herself, Sue races home and uses what remains of the original serum, in the desperate hope of recreating the perfect version of herself, but it’s not possible. She creates a monstrous hybrid of Elizabeth and Sue, and heads out on stage to host the show, to the shock of the audience. The creature’s head explodes and blood goes everywhere, covering the audience and the whole auditorium. The monster escapes outside before her whole body explodes on the ground. All that remains is Elizabeth’s face sliding across the floor on the last bit of the monstrous gunk. She crawls toward her star on the Hollywood Walk of Fame and melts and dissolves. The next day, the bloody leftovers of the monster are cleaned up by a floor scrubber.
