Instead, veer right from the Director’s Office, climb the stairs to the third floor, and there, you’ll need to smash a window to gain entry to Exam Room 5. On the desk inside, secure the 1F Inner Ward Key. Proceed into the X-Ray Room where you’ll find a Radiograph of a Pelvis and an X-Ray Viewer Puzzle. This puzzle requires four Radiographs in total, so your next mission is to locate the remaining Radiograph.
Where To Find The Mold Remover In Silent Hill 2 Remake
Make your way to the Ground Floor Inner Courtyard, then veer left towards the Pantry. Inside the Pantry, you’ll find the Mold Remover – make sure to grab it.

Go to the Pharmacy by navigating through the narrow opening in the Dayroom and proceed into the Garden. Avoid the wandering statues and make your way to the Pool area. The double doors at the end of the Pool lead you to the Director’s Office, where you can find the Keypad Combination Note displayed on the wall. Return to the Nurses Station and input either the code 3578 (for Light and Standard Mode) or 7456 (for Hard Mode). Inside the Station, you can open the door to the Treatment Room, where a decayed X-ray can be found inside the container.
Combine the Modly Radiograph and Mold Remover to get the Cleaned Up Radiograph. Return to the X-Ray Room, and you can solve the puzzle. You need to place all the Radiographs and move and rotate them to match the markings. This will reveal the numbers 4, 37, and 12.

Solving the Room D1 Combination Lock
Once you’ve figured out the puzzle, head over to Room D1 and engage with the Combination Lock. The rotations are clearly indicated on the door, so the combination for this lock is as follows:
