Why Justin Chambers Left Grey’s Anatomy (& When)

For 22 seasons, Grey’s Anatomy, created by Shonda Rhimes, has followed the lives of doctors and nurses at Grey Sloan Memorial Hospital (formerly Seattle Grace). The show began by introducing viewers to Meredith Grey and her fellow medical interns: Cristina Yang, Izzie Stevens, George O’Malley, and Alex Karev.

I have to admit, Alex Karev was a character you just couldn’t ignore on Grey’s Anatomy – some people adored him, others loved to root against him! I remember when the show was really hitting its stride, it was amazing to watch Alex grow from this brash, kinda self-centered intern into a truly gifted doctor, especially with kids. So, when Justin Chambers suddenly left during Season 16, it really shocked everyone. I always wondered exactly when and why he decided to leave the show.

When Did Justin Chambers’ Alex Karev Leave Grey’s Anatomy?

During the first few seasons of Grey’s Anatomy, Alex and Izzie had a complicated relationship. They eventually became a committed couple in Season 5, culminating in their wedding in the episode “What a Difference a Day Makes,” which was also the show’s 100th episode. Later, Izzie was diagnosed with cancer and, after a serious mistake with a patient, was fired by Richard Webber in Season 6. She moved away, and Alex focused on becoming a skilled pediatrician, eventually finding love with OB/GYN Jo Wilson.

Jo and Alex have a dramatic relationship that lasts for over four years, beginning in Season 10. It ends when Alex decides to leave Grey Sloan Memorial Hospital in Season 16, Episode 16, titled “Leave a Light On.” The episode starts with a nostalgic look back at the interns’ first day, featuring Richard Webber’s famous speech about the challenges of intern life. As this footage ends, Jo, Miranda Bailey, and Meredith are shown reading letters from Alex, revealing he’s already left to be with Izzie and their children.

A touching montage shows Alex’s journey over fifteen years, highlighting key moments like his relationships with Izzie, Jo, and Meredith. Justin Chambers narrates, explaining that he reconnected with Izzie and learned she beat cancer. She had previously frozen their embryos and recently welcomed twins, Alexis and Eli. While Chambers provides the voice-over, he doesn’t appear in any new scenes, making his last on-screen moment in Season 16, Episode 8. The montage is emotional and nostalgic, ultimately reinforcing the idea that Alex and Izzie were meant to be together.

Why Did Justin Chambers Leave Grey’s Anatomy?

Alex Karev’s departure from Grey’s Anatomy sparked a lot of debate among viewers. Many felt the way his exit was handled – through a letter read on-screen – didn’t do justice to a character who’d been on the show for so long. However, the writers faced a difficult situation: the actor who played Alex, Justin Chambers, had already left before the announcement, meaning they had to explain his character’s absence in a later episode. Chambers officially announced his departure to Deadline in 2020, stating:

I’ve been wanting to take on different kinds of acting roles and explore new career paths for a while now. With my 50th birthday approaching and feeling incredibly grateful for my amazing wife and five children, I feel it’s the right moment to do that.

Let me tell you, the outpouring of gratitude after T.R. Knight’s exit was really something. Not only did he thank the whole team at ABC and the creative forces behind Grey’s Anatomy – Shonda Rhimes, Ellen Pompeo, Chandra Wilson, James Pickens, and the entire cast and crew, past and present – but he also gave a shout-out to the fans for what he called an ‘extraordinary’ journey. It’s been amazing to see characters come back to the show, even after they’ve been written off – remember that incredibly emotional dream sequence bringing back Derek Shepherd, George O’Malley, Lexie Grey, and Mark Sloan? It really tugged at the heartstrings. But, notably, T.R. Knight himself hasn’t been one of those returning faces.

Grey’s Anatomy is Now in Its 22nd Season With More Long-Term Stars Exiting

Season 22 of Grey’s Anatomy premiered in October 2025, bringing back fan favorites Levi Schmitt and Mika Yasuda, who were originally interns in seasons 14 and 19. However, this season also said goodbye to Owen Hunt and Teddy Altman. Kevin McKidd, who plays Owen – an army trauma surgeon who joined the show in season 5 – has explored themes of PTSD, complicated relationships with Cristina and Amelia Shepherd, and a strong yearning for a family through his character. McKidd also directed 48 episodes of Grey’s Anatomy, leaving a significant mark behind the scenes.

Teddy Altman joined the cast of Grey’s Anatomy in Season 6, replacing Erica Hahn. With a background in the military, she quickly became a valuable asset in the hospital’s trauma department and began a complicated, on-again, off-again relationship with Owen Hunt. Both Teddy and Owen are expected to make their final appearances in the Season 22 finale, airing on May 7, 2026. While the actors have expressed appreciation for the show, reports suggest they are leaving because they weren’t happy with the direction their characters’ stories were taking.

Just because Teddy and Owen are currently off-screen doesn’t mean their story is over. Kim Raver, who plays Teddy, left Grey’s Anatomy before, returning several seasons later to resume her relationship with Owen. It’s possible the show could find a way to bring them back to Grey Sloan Memorial Hospital in the future.
